We’re delighted to welcome you as a new member of our staff and want to ensure you have a smooth and successful start. As part of the hiring process, there are several required documents and training modules that need to be completed.
Please submit copies of all completed documents and certificates to the Parish Rectory, addressed to “New Employee.”
When completing your clearances, be sure to select the employment-related options. We also recommend keeping copies of your receipts, as you may submit them for reimbursement.
Safe Environment Clearances
1 ~ PA State Police Criminal Record Check
(this must be renewed every 5 years)
This background check must be completed online, and results are typically available within minutes.
As soon as your results appear, click on the CONTROL # and then select CERTIFICATION FORM — we require the official certificate, not the summary or record check pages.
Please print the certificate and submit a copy to the Parish Office at the Rectory without delay.
The clearance fee is $20, payable by card at the time of your request. Be sure to keep your receipt, as it can be submitted for full reimbursement.

PA State Criminal Record Check
2 ~ PA Department of Public Welfare Child Abuse Clearance Check
(must be renewed every 5 years)
This clearance requires a $13.00 fee, which can be paid immediately online using a card.
If you prefer to mail in the request, include a $13.00 money order or check made payable to:
PENNSYLVANIA DEPARTMENT OF HUMAN SERVICES.
⚠️ Do NOT send cash.
Be sure to keep your receipt—you may submit it for full reimbursement.
Once you receive the certificate by email, print it and deliver a copy to the Parish Office at the Rectory promptly. If you receive it via mail, please bring it to the Parish Office.

Mandated Reporting of Suspected Child Abuse (online) Provides an overview of Archdiocesan reporting procedures for suspected child abuse and Pennsylvania Child Welfare Laws.
This training is mandatory for all the employees. This training is available online through the Office for Child and Youth Protection website under “Training Institute” THIS TRAINING IS LIVE AGAIN AS OF 6/30/2021

This form needs to be completed by the employee with his/her personal information and an emergency contact.
2 ~ I - 9 Form ~ Employment Elegibility Verification
This form is used to document verification of the identity and employment authorization of each employee.

This form is used for the employee to choose an additional withholding from the paycheck for the taxes.

This form is to be used by employers and/or taxpayers to report essential information for the collection and distribution of Local Earned Income Taxes
5 ~ Direct Deposit Form
We need this form completed to deposit your wages into your bank account.

If the employee is a minor, this form needs to be completed by his/her parents.
